Section 1: Confirmation of Member’s Understanding
The purpose of Section 1 of the Membership Renewal Form is to confirm that you understand that:
- You are to provide truthful information in your Membership Renewal Form and any supporting documents
- There may be consequences for providing false or misleading information in your Membership Renewal Form or in relation to your Membership Renewal Form
There are three statements. To confirm your understanding of the statements and agree to the terms of the Membership Renewal Form,check the box for each statement. Also type/print your name and indicate the date. The College treats this entry as your signature.
