Requirement 2: Providing Supporting Documents

The second requirement for changing your membership information is to gather the supporting documents that must be submitted along with your Change of Information Form.


For changes of information reported under the following sections of the Change of Information Form, you must submit supporting documents as proof of the change you are making.
