Requirement 1: Completing the Change of Information Form

​​​​The first requirement for changing your membership information is to complete and submit the Change of Information Form.

​​Are you completing the correct version of the Change of Information Form?

  • The correct version of the form is the version that is valid until June 30, 2018. The validity date is printed on the bottom of each page of the Change of Information Form. 

  • Do not use an​y other version of the Change of Information Form that you may have. The College does not accept versions of forms that are submitted after their expiry date.


There are six sections of the Change of Information Form. The next several web pages provide information about each section, including instructions about what supporting documentation needs to be submitted along with your Change of Information Form. ​​

  • Sections 1 and 6 of the form are mandatory in all circumstances

  • Complete sections 2, 3, 4, and/or 5 as applicable for your circumstances
