The purpose of Section 1 of the General Application Form is to confirm that you understand that:
There are three statements. To confirm your understanding of the statements and to agree to the terms of the
General Application Form, check the box for each statement. Also type/print your name and indicate the date. The College treats this entry as your signature.
In the first part of Section 2, you are to provide information about any previous applications you made to the College.*
